By providing physical therapy services in the home, patients can receive individualized attention, reduce the risk of falls and other accidents, and learn how to manage their condition more effectively. 

Home health physical therapy is provided by licensed and trained physical therapists who work closely with patients, their families, and other healthcare providers to develop a personalized treatment plan that meets the patient's specific needs and goals.

SERVICES

• Assessment: The first step in the skilled nursing process is to assess the patient's physical abilities, range of motion, pain level, and other factors that may impact their ability to move and function. This assessment helps the physical therapist develop a comprehensive treatment plan tailored to the patient's unique needs.

• Treatment planning: Based on the assessment, the physical therapist will develop a treatment plan that outlines the specific exercises, activities, and interventions needed to help the patient regain mobility, reduce pain, and improve overall function. This treatment plan is developed in collaboration with the patient, their family, and other healthcare providers.
• Implementation: Once the treatment plan is developed, the physical therapist will begin implementing the interventions and services outlined in the plan. This may include exercises, stretches, massage, and other specialized physical therapy techniques.
• Evaluation: As treatment is provided, the physical therapist will regularly evaluate the patient's progress and make adjustments to the treatment plan as needed. This may involve measuring the patient's range of motion, assessing their pain level, and communicating with other healthcare providers to ensure coordinated care.

Our professional team is available 24 hours per day, 7 days per week* During hours when the office is closed and during the weekends and holidays, the on-call coordinator is available by telephone 24 hours per day.
